[mambo] New template missing index.html

Thank you very much for helping to clear this up.  I would never have
figured that out as the install identifies itself as:

Mambo Version:   	 Mambo 4.5.2 CVS [Die Hard] 20/10/2004 01:31 GMT

on the system information page of the Admin interface.

I will download again!
