Are you wanting to charge them for the card? If that's the case you
could roll with QuickCart from Rocket Theme:

If not, then you're just submitting a form and capturing it in
database right? Could use any of the form extensions to do that easily
enough. In situations like that I usually write a cronjob that fires
off a CSV export to an email address for human processing.
