Something like this might be a start:

$str = preg_replace(
   array(
     '/\[\[([^|\]]+)\]\]/U',
     '/\[\[([^|\]]+)|([^\]]+)\]\]/U',
   ),
   array(
     '<a href="$1">$1</a>',
     '<a href="$1">$2</a>',
   ),
   $str
);
