I don't know if you're just looking just for public web sites but the 
Transportation and Referral Management Systems (TRMS) I built for them 
is rolling out to all 17 CHF programs around the country.  It's PHP5 + 
PostgreSQL.  I'm about to start a new one for childhood diabetes.
