The only things I can think of are make sure file permissions are 
correct. Not only the file owner and group but that the attributes 
(read, write, execute) are correct also. Then make sure that the 
permissions are correct for the include folder. I know this sounds petty 
but is the file in the folder?

That's all I can think of off hand.
