[joomla] Fill out form, then get a PDF downloaded?

David,

I've used the Documents Seller extension AKA DMS, from joomdonationcom. 
What is lacks in refinement, it makes up in features.  It's $40 but you 
get a pretty complete system for distributing documents, free or for 
money, so your visitor info is covered in the cart part, and even manage 
uploads. The doc can be an immediate download, or emailed. Time or 
number of downloads is settable as well.
